The following pages contain some pre-screening questions to determine whether you may be eligible to take part in the AMPLITUDE clinical research study. Vertex Pharmaceuticals Incorporated (“Vertex”) is the sponsor of this study and has engaged We Are COUCH Ltd (“COUCH Health”, “we”, or “us”) as a service provider to assist with clinical trial recruitment by collecting pre-screening information for the study through this website. The categories of information collected via this questionnaire include information about your health (to determine eligibility) as well as contact information (in the event you choose to share it).

We will use this information only to determine whether you are eligible for this research study and, if you consent, to inform you of potential Vertex research studies in the future. The pre-screener information you submit online will not be shared with Vertex in any way that could identify you. You will also have the option to delete the information after you complete the pre-screening process.

If your answers to the questionnaire indicate you may be eligible for this study and you elect to share your contact information, we will share your information with Circuit Clinical or another service provider to Vertex on this study, who will contact you to do a full pre-screening. Based on the results of that screening, and with your permission, your information would be shared with a research site. Even if you are not found to be eligible for this study, you will have the option for COUCH Health to keep your information on file for a period of time that you elect so that you can be contacted if the eligibility criteria change for the AMPLITUDE study or for future Vertex studies.
